Hazina Sacco Loan Application Form 2026: How to Fill It

Updated 2 November 2026

The Hazina Sacco loan application form is a free PDF on the Sacco’s downloads page (hazinasacco.or.ke/downloads), listed under BOSA downloads as “Loan Form_Revised 2026”. You fill in every section, attach two recent original payslips and a copy of your ID (plus a KRA PIN, bank statements or collateral documents where they apply), get guarantors and your employer to sign, then hand it in at a Hazina office or send it to loans@hazinasacco.or.ke. Incomplete forms are returned unconsidered, so here is how to get it right first time.

Where to download the Hazina Sacco loan form

  1. Open hazinasacco.or.ke/downloads.
  2. Under BOSA Downloads, choose Loan Form_Revised 2026 (it is also listed under General Downloads as “Loan Application Form Revised 2026”). Read the “Circular – Revised Loan Form” on the same page.
  3. For a FOSA salary advance or a Hisa loan, use the separate FOSA Salary Advance Application Form or HISA Loan Application Form under FOSA Downloads.
  4. If you are a private or contract member, also download the Guarantors Loan Consent Form.

Print single-sided and fill in clearly in ink. Mobile loans (Flash Advance, Mobi Loan, Dividend Advance) do not need a paper form; you apply on *850# or the Hazina Sacco app.

Document Who needs it
2 months’ original current payslips (not older than two months) Every applicant
Copy of national ID Every applicant
KRA PIN Where applicable (always if you offer collateral)
6 months’ bank statement and a standing order Private members (not on payroll check-off)
Guarantor consent form Private members and contract or temporary staff
Copy of appointment letter Contract or temporary staff
Original logbook, title deed or share/fixed-deposit statement If you secure the loan with collateral; jointly owned property also needs a consent letter

How to fill the form, section by section

The form we reviewed runs from section A to J. Section numbering in the 2026 revision may differ slightly, but the content is the same kind of information.

  • A. Requirements and instructions. Read them; the form says applicants must be familiar with the current credit policy.
  • B. Personal information. Full name, current and home address (county, sub-county, location, village), personal/employment number, membership number, mobile, email, employer and work station, terms of employment, age, ID number and KRA PIN. Self-employed members add the business name and location.
  • C. Type of loan. Tick one: BOSA (Super, Normal/Top-up, Daraja, Jibu, School Fees, College Fees, Emergency/Top-up, Tech Loan) or FOSA (Karibu, HSF Special Advance, HSF Okoa). The form notes all interest is on a reducing balance.
  • D. Loan amount. Amount in figures and words, period in months and the monthly instalment.
  • E. Purpose. Tick the purpose (education, land and housing, trade, agriculture, medical, buying a car, clearing another loan and so on). This is used for regulatory reporting.
  • F. Funds transfer. Loans are paid into your Hazina FOSA account. If you want the money moved to your bank, give the account name, number, bank and branch.
  • G. Declaration. Sign and have a witness sign. You consent to CRB checks and to deductions from salary.
  • H. Guarantors and collateral. Each guarantor, who must be a Hazina member, writes their name, ID, employer, deposits and the amount guaranteed, and signs before a witness. Or give collateral details (title, logbook, shares).
  • I. Employer’s undertaking. HR confirms the deduction fits the one-third rule and that they will deduct repayments.
  • J. Official use. Leave blank; the loans officer records charges, the net amount and approval.

How much can you borrow?

Hazina lends up to 5 times your deposits (6 times on the Super Loan), up to KSh 40 million on BOSA loans, at 1% to 1.2% a month reducing. Check-off members qualify for a Jibu Loan (up to KSh 300,000) after the first payroll remittance; others after four months of steady contributions. Compare Sacco and bank borrowing in our bank vs Sacco loans guide, and estimate the instalment for section D with the Hazina Sacco loan calculator.

Submitting and processing

  1. Hand the completed form to any Hazina branch or the head office at Hazina Sacco Place, Kibera Road off Ngong Road, Nairobi, or scan and email it to loans@hazinasacco.or.ke.
  2. The form says loans are processed in the order they are received.
  3. Money goes to your FOSA account, then to your bank if you filled section F.
  4. If your loan is not recovered through payroll, pay on time by cash or Paybill 850436 to avoid arrears interest, penalties and default. Non check-off members must set up a standing order before disbursement.

Loans desk: 0712 149 631. Head office: 020 271 9098. More branches in our Hazina Sacco contacts list.

Common reasons forms are returned

  • Blank fields in personal information or the loan section.
  • Payslips older than two months, or photocopies instead of originals.
  • Guarantors’ deposits do not cover the amount guaranteed.
  • Instalment breaks the one-third rule, so HR will not sign.
  • A negative CRB listing; check yours first with our CRB status guide.
